OBJECTIVES: The main objectives of the study were: 1) To obtain health valuations for the EQ-5D set of health states from an Argentinean general population sample using the Time Trade Off (TTO) valuation technique; and 2) To describe the general population health status. METHODS: Cross-sectional study undertaken in consecutive subjects attending four primary care centers in large and small urban areas, with quota sampling according to demographic characteristics.   READY-TO-USE BOXES WITH MAGNESIUM SULFATE TREATMENT FOR ECLAMPSIA TO REDUCE MATERNAL MORTALITY / Cajas de Sulfato de Magnesio

This project will measure the effectiveness of a strategy to facilitate the use of magnesium sulphate to women with severe pre-eclampsia and eclampsia (PE-E) at all levels of care throughout the use of a ready-to-use box containing all the elements needed for the first 24 hours treatments, instructions for the administration and guidelines for management and monitoring. It will be implemented at large scale in developing countries and evaluated through an operational research design.  It is expected that this strategy will facilitate skilled attendants the selection of the effective treatment and its use in a timely and appropriate manner, reducing PE-E related maternal mortality.

The quality of health care, refers to the levels of excellence that characterize the care provided. An important indicator of quality of care is the satisfaction of people using the services. The satisfaction with care is associated with greater adherence to treatment and with a better and greater use of health services. This study aims to develop a set of tools to measure user satisfaction with care during pregnancy and childbirth, their relationship to the fulfillment of expectations and their relationship to the quality of care received by patients attending the Network Perinatal Centers in the metropolitan area of Buenos Aires and South of the Province of Buenos Aires. As a result of this study to be carried out two surveys to apply to women: A) Satisfaction Survey with regard to prenatal care controls, and B) Satisfaction Survey with regard to the attention of labor, delivery and postpartum immediately. This toolkit as a whole or separately may be used in care during pregnancy and childbirth public or private Argentina and elsewhere in Latin America.

  PREVALENCE OF UNINTENDED PREGNANCY IN LATIN AMERICA AND THE CARIBBEAN AND IT’S ASSOCIATION WITH MATERNAL AND FETAL OUTCOMES AND BEHAVIORS / Embarazo no deseado

This is an investigation to determine the prevalence of unintended pregnancy in Latin America and to determine if unintended pregnancy is associated with poor perinatal, maternal and child outcomes. An epidemiologic study will be conducted in the associated centers of IECS in countries with high rate of institutional deliveries (over 90%). The first hypothesis is that the prevalence of unintended pregnancies in these countries is similar to that of other Latin American countries, between 30 and 50%. The second hypothesis to be tested is that as compared to intended pregnancy, unintended pregnancies increase the risk of perinatal death, childhood death in the first year of life, low birth weight, preterm birth, intrauterine growth retardation, low Apgar scores, alcohol consumption during pregnancy, smoking during pregnancy, not breastfeeding, and delayed initiation of prenatal care.

  WOMAN’S RIGHT TO A CHILDBIRTH WITH EQUITY AND QUALITY. Educative intervention at community level / Empoderamiento de mujeres para mejor asistencia del embarazo y parto

This is a project to design, apply and evaluate an educative intervention at community level, directed to empower women through knowledge and to train health workers. The purpose is to modify a set of medical procedures commonly used during childbirth, that have been shown to be either ineffective, or even harmful to women, and introduce other procedures that have been proven to be beneficial.

  EARLY VERSUS DELAYED UMBILICAL CORD CLAMPING DURING THE THIRD STAGE OF LABOR IN TERM INFANTS / Ligadura de cordón

This is a protocol for a two arms randomized controlled clinical trial in which women having a term vaginal delivery will be randomized to one of two interventions: early (<10 seconds) or delayed umbilical cord clamping (>3 minutes) The trial will test the hypothesis that in term newborns, born vaginally, delaying the clamping of the cord at the time of birth will result in a lower incidence of anemia at 3 month of age when compared with immediate cord clamping, without increasing neonatal morbidity and mortality. Two pre-specified subgroup analyses will be conducted, to assess if the study effect is modified by maternal anemia and the use of prophylactic oxytocics. In summary, this is a protocol for a large multi-centric trial that should provide conclusive evidence about the best timing of umbilical cord clamping on neonatal outcomes in term infants. Such evidence would be critical to guide the assistance of deliveries around the world in both hospitals and community settings.

  Maternal and infant health in Latin America. Improving pregnancy outcome in six low- and middle income countries. / Mejoría de la salud materna y perinatal en 6 países de Latinoamérica basado en el trabajo en Red.

This project’s main objective is directed to improve maternal morbidity and mortality, as well as perinatal and neonatal health in Latin American countries collaborating in IECS Network. The specific objectives of the proposal are: a) training of professionals in the six centers and their associated clinics, in evidence-based perinatal and maternal health, epidemiologic surveillance and research methodology; b) establishment in each of the associated clinics of the centers of the six countries of a common system of epidemiologic surveillance of maternal morbidity and mortality with the Perinatal Information System; c) identification of the associated factors of maternal and perinatal morbidity and mortality; d) asses quality of care by measuring the use of evidence-based practices of care; e) improve quality of care by promoting the use of evidence-based practices of care, including skills training (ALSO) f) design research and care strategies to improve maternal and perinatal condition in the region.

  Prevalence of Unintended Pregnancy in Latin America and it’s Association with Maternal and Fetal Outcomes and Behaviors / Prevalencia de embarazo no planeado en América Latina y su asociación con resultados maternos, fetales e infantiles

The main objective of the study is to determine the prevalence of unplanned pregnancy in hospitals in Latin America and whether the unplanned pregnancy is associated with perinatal death, death of the child within the first year of life, low birthweight, preterm delivery, intrauterine growth retardation, low Apgar score at birth, alcohol consumption during pregnancy, smoking during pregnancy, not breast-feeding, or delay in the initiation of prenatal care.

  Simple intervention to reduce episiotomy rates: A cluster randomized controlled trial / Reducción de Episiotomías con guías y recordatorios

This proposal intends to evaluate the efficacy and effectiveness of a simple, inexpensive and easy to implement intervention to reduce episiotomy rates at health facilities. We propose a cluster randomized controlled trial to evaluate an intervention consisting in the delivery of guidelines and the placing of reminders at health facilities assigned to the intervention group, during a one-year period.
