Summary:

The project aims to strengthen health services at the primary level of care through the implementation of innovative strategies based on mHealth and social networks in order to improve the identification, management and monitoring of adults with high risk type 2 diabetes who They receive care in primary care centers (CAPS) in the province of Mendoza and thus reduce the incidence of type 2 diabetes in this population.

This is a hybrid effectiveness-implementation design. 10 provincial CAPS will be included; five will be randomized to receive the intervention and five to continue with usual care. 40 people will be included per CAPS with age ≥35 years and FINDRISC score ≥13 (total 400 participants, 200 participants per group). In the intervention group, health agents will be trained in the use of an app that contains FINDRISC integrated into an electronic scheduling system so that they can schedule an appointment at the CAPS. Participants will receive SMS reminders of the shifts and educational messages to promote healthy lifestyles. In the control group, usual care will be implemented. In both groups, workshops will be implemented to modify lifestyles for people with prediabetes.
